this application-obligation Form and payed the preliminary sum to the account stated
below, that means we entered the agreement relations with You, and we have mutual
obligations. In this case You'll get our message mail, where the receipt of preliminary
sum will be confirmed and we'll send You our obligation to conduct the search of wanted
person. The work on the search starts immediately after receiving the preliminary sum on
stated account. We inform You that the sum You paid is not final. The size of all price of
our service will be stipulated with You in addition, with the consideration of our
expences and the result of our work.

activities.
Application-obligation
I ask You to help me in the search of my relative (friend, familiar), who in my
opinion, lives in Ukraine, and send me an information which will be found during the
search. I guarantee that the search person is really my relative (familiar) and in the
case he (she) will be found or getting information about him (her) or in the case of
determination of his (her) death, I will pay stipulated in addition sum for the work,
connected with his (her) search. I confirm that the receipt of information wanted person
doesn't have the purpose to do any property or private harm for him (her), and the
information won't be used me in the harm of the protected human rights, another rights and
interests of the third persons and the State of Ukraine. Being the conscientious acquirer
all information about wanted person, given to me by People Search Department or other
representative person, and in the case of using by my in any form. I pledge myself to
refer that this information was get with the help of People Search Department.
